What happens when you submit the form

The form is intended for enquiries and arranging a consultation. We ask for your full name and telephone number. You can also provide an email address, choose a topic and leave a short message. Your request is stored in the website system and sent to the practice by email. Do not use this form to send medical records, your Polish personal identification number or detailed health information. In an urgent situation, do not wait for a reply through the form.

If you enquire about an appointment or service, we process your information to take steps before entering into a contract, under Article 6(1)(b) of the GDPR. For other correspondence, the basis is our legitimate interest in responding to your enquiry, under Article 6(1)(f). Confirming that you have read this policy is not consent to marketing. Providing information is voluntary, but we cannot handle a request for contact without the required details.

Security and access

Authorised practice staff and providers of hosting, email and technical maintenance may access the information to the extent necessary to operate the website and respond to you. Server logs may contain your IP address, connection time and browser information. These records support security and the detection of misuse, on the basis of the controller’s legitimate interests.

We retain correspondence for as long as needed to deal with the enquiry. After that, we keep it only where a legal obligation or the protection of legal claims justifies retention. The website form does not replace medical records, which are subject to separate rules.

Cookies and external content

Necessary mechanisms support the website, the form and your language and privacy preferences. Optional external content, including Google Maps, is enabled after consent. You can change your choice through the cookie settings button. The website does not currently activate analytics or marketing tools.

Once a map is enabled, its provider may receive connection information, including your IP address. Using Google services may involve processing outside the European Economic Area. See Google’s privacy policy for details. Opening a social media link takes you to a website managed by that provider.

Your rights

You can request access, correction, erasure or restriction of processing. Where the law provides for it, you also have rights to data portability and to object to processing based on legitimate interests. You may withdraw consent for optional features without affecting the lawfulness of earlier processing. We do not use the form to make decisions with legal effects solely by automated means.

If you believe your information is being handled unlawfully, you can complain to the President of the Polish Personal Data Protection Office. Guidance on making a complaint is available on the UODO website.
